Terms and Conditions
CONFIDENTIALITY
The information you share with us is strictly confidential and will be handled with care by the Make A Melody team. Your details will be passed on to one of our counsellors, who will conduct an assessment with you. During this assessment, you and your counsellor will discuss whether counselling is the right approach for your current symptoms or issues. If third-party professionals are involved in your care, Make A Melody may seek their input to ensure the safety and suitability of counselling for you. We will not proceed with counselling if it is deemed inappropriate, but we will work with you to find the best form of support for your needs.
Confidentiality may be breached in the following situations:
- There is a risk of you harming yourself or being harmed
- There is a risk of harm to another person
- There is a risk of a serious crime being committed
All counsellors discuss their work with a clinical supervisor, but any identifiable information is withheld, and supervisors are also bound by confidentiality.
EMERGENCY CONTACT
In the event of an emergency, we may need to contact your next of kin if you have provided their details. If no next of kin details are provided, your GP will serve as your emergency contact.
If GP details or emergency contact information are withheld, Make A Melody reserves the right to decline offering you counselling, as it is not safe or ethical to provide counselling without a key contact responsible for your well-being and health.
CANCELLATIONS
Please provide at least 24 hours’ notice (where possible) if you need to cancel an appointment, or a £5 administration fee may be charged. Links Counselling will provide the same notice (where possible) if we need to cancel. If you cancel two consecutive appointments, you will be placed back on our waiting list.
DATA RIGHTS
Any data submitted through our referral form will be securely stored in our centralised database and cloud system. The information collected is used solely to contact you and arrange an assessment. Submitting a referral form does not automatically make you a client of Make A Melody. If you do not become a client, we will not retain any additional information about you.
By submitting a referral form, you consent to Make A Melody contacting you via the method you provided. We will not share your details with any unauthorised third party and will keep all data confidential.
DATA SHARING
By submitting your referral form to Make A Melody, you agree that we may contact the most appropriate representative at your GP surgery, who holds medical and clinical responsibility for your mental health and well-being. In many cases, information will be shared on a ‘need-to-know’ basis regarding your counselling referral, an evaluation of its benefits, and an assessment of ongoing needs if appropriate. Sharing this confidential information with your GP surgery is considered good and ethical practice to enhance the quality of care you receive. All data shared will be GDPR-compliant.
